Pengaruh Terapi Latihan Postur dan Mobilisasi Shoulder Girdle pada Kemampuan Fungsional Tangan Anak Cerebral Palsy

Abstrak

Cerebral palsy (CP) spastik diplegia ditandai dengan gangguan motorik dominan pada ekstremitas bawah, namun sering kali disertai penurunan kemampuan fungsional tangan akibat ketidakstabilan postur dan kontrol shoulder girdle yang buruk. Stabilitas postur merupakan prasyarat mutlak untuk mobilitas tangan dan kaki yang efektif. Penelitian ini bertujuan untuk menganalisis pengaruh kombinasi terapi latihan postur dan mobilisasi shoulder girdle terhadap peningkatan kemampuan fungsional tangan pada anak CP spastik diplegia usia 3-5 tahun. Penelitian ini menggunakan desain one group pre-test and post-test. Sampel terdiri dari 12 anak CP spastik diplegia (8 laki-laki dan 4 perempuan) di YPCP Surabaya 4,2±0,9 tahun yang dipilih melalui teknik purposive sampling. Intervensi dilakukan selama 8 minggu dengan frekuensi 3 kali seminggu. Instrumen pengukuran fungsional tangan menggunakan Melbourne Assessment 2 (MA2). Analisis data dilakukan menggunakan uji normalitas Shapiro-Wilk dan uji inferensial paired t-test. Hasil penelitian menunjukkan peningkatan rerata skor fungsional tangan MA2 dari 5,2 (pre-test) menjadi 7,6 (post-test) atau meningkat 46%. Uji paired t-test menunjukkan nilai signifikan p<0,001. Kesimpulan: terapi latihan postur dan mobilisasi shoulder girdle meningkatkan kemampuan fungsional tangan pada anak CP spastik diplegia.
